Chloride, Serum (Cl⁻) is the major extracellular anion and plays an essential role in maintaining fluid balance, osmotic pressure, and acid–base equilibrium in the body. The serum chloride test measures the concentration of chloride ions in blood and is commonly performed using the ion selective electrode (ISE) method. It is routinely used to assess electrolyte balance, hydration status, and metabolic acid–base disorders, and to support the evaluation of renal function. Altered chloride levels may be associated with conditions such as dehydration, prolonged vomiting or diarrhea, renal disorders, metabolic acidosis or alkalosis, and disorders affecting fluid balance. Measurement of serum chloride is particularly important in critically ill patients and is often interpreted alongside sodium, potassium, and bicarbonate to provide a comprehensive assessment of electrolyte and acid–base status.
